I have used a variation of this since I first heard it from a friend at school when I was about fifteen (~1959). He usually offered it as a response when someone said 'Thank you' and that is how I use it now. I have no idea where he got it from. So what you would get is a person saying "Thank you" and this is responded to by "kind sir, she said, waving her wooden leg gaily in the air". It sort of suggests a parody on poor writing e.g "It was a dark and stormy night..." Anyway, I still use it a lot.
