Five-foot-four is what I've read. Robert Blake refers to it in a scene from an excellent film called "Electraglide in Blue" (early 70's, I think).

Not just boxes for close-ups, but apparently they also dug trenches for his leading ladies to walk in to disguise the height differences. Knowing that, it becomes pretty darned obvious watching some of his films.
