After Katherine Hepburn has helped pick all the leaches off him in "African Queen", Humphrey Bogart has to get back into the leech infested waters if the boat is ever going to get through the swamp.

I have bad memories of a childhood vacation at Leech Lake Minnesota, so that scene is a metaphor for having the courage to face unpleasant tasks that must be done.
