Found the Cornish hymn when clearing out a handbag (yes - it happens sometimes).

The hymn-sheet they gave us doesn't say who wrote this, and it gives "Crugyer" as the tune. I remember thinking the tune was vaguely familiar but I can't find it either in the Cyberhymns or in general Googling.

By accident, we did end up finding the old St Madern chapel and holywell when we were in Cornwall. A real 'find'.
