Dear Mark — Hello again!

Just after our last "talk" (several years ago), I had to change my e-mail address in trying to overcome some computer problems around the time I moved house (problems which in the end did take years to clear up)! Thereafter, I tried to e-mail you, but the messages always bounced ...

So (since it seems I've found you again) I just wanted to:

/1/ ... thank you for having permitted me to use your "Formulary Song" in the handwriting classes I give MDs (as a penmanship-practice piece rather more workplace-relevant than "The quick brown fox jumps over the lazy dog"),

/2/ ... let you know that the MDs respond well to it,

/3/ ask again for the precise NEJM citation of the "Formulary Song" (which it seems I lost in my house-move),

and
