Glad to see things seem a lot better.

"Care in the community" all too often means "neglect in isolation".   Where it actually lives up to what it says, it ought to mean all the kind of stuff you say it can'f, especially as the right kind of help as and when and where you need it. But it's a term that has been misused so much that it's time it was pensioned off.
