I always thought it was just about a vet with PTSD self-medicating with booze and who knows what else. It reminds of hanging out with 2 of my   high school friends who had done tours in Viet Nam. We got into some extreme booze and drug-fueled behavior.
I think this is one of the Pogues best songs and one of Shane's best lyrics.
