Chords Req for Grimsby Lads

Can anyone supply me with the chords for Here's To The Grimsby Lads
for both the verse and chorus. I know the chorus sounds very similar to the verse but I think I here some subtle differences on the John Conolly CD I have.

Preferably in D, but I might just be able to get away with C!

It's not just the tune he changes he also tinkers with the words and has a habit of adding bits!

Sings/writes some fantastic songs tho' eh!

By the way, where might I get "The Singing River" book.
