I saw your query a few weeks ago, and was determined to help you, this being a memorable song for me. I now have some answers for you, with help of an excellent book "That Old Time Rock & Roll: A Chronicle of an Era", by Richard Aquila, 1989. The song "Last Kiss" was released in 1964, and was performed by Frank J. Wilson and the Cavaliers. The lyrics given by Barry Finn above seem correct, but there are more verses, though. Can't help you there. As far as the chords go, C, A minor, F, and G are used for the entire song, including the chorus.
