My impression is that 'Hey' as a greeting is very much an American thing, and a somewhat regional thing at that. I would be interested to know if it has spread, or if it has long been established in other places. In Canada, it would only be a greeting if a person were taken by surprise, and would be followed by something considered more cordial, e.g., "Hey! How're you doing?"
