What the chords above written music often indicate is that the music publisher got some damn piano player to write down what he/she was playing without any regard to the principles of the guitar (hence the carefully transcribed pieces in E flat and A flat). The B/D might represent a passing note in the bass that looks wrong in the static chord diagram.
