In Massachusetts, "milkshakes contain no ice cream; if you want ice-cream added, you order a "frappe." The drink made with milk, ice cream, and syrup was called a "cabinet" in Rhode Island, and was called a "frosted" in Connecticut, where I grew up. The rest of the country called them "milkshakes" or perhaps "malts," though a malted milk required the addition of malt when I grew up. These regional distinctions have probbaly become homogenized as milk in the new era where nobody is from anywhere in particular.
