Seeing as how long ago this thread started, maybe it really is a "Fake Book"!

It did finally come out - in August, 2015. The title is Rise Again, and the publisher is Hal Leonard because Sing Out! was not able to publish the book. And one Associate Editor was a guy named Joe Offer.
