Stick on labels - even ones intended for use on CDs - may unbalance the disk and cause problems beside coming loose. The only real recommendation one can offer from the info you have given is to take to to a professional repairer. As per various other threads here on 'Walmart' though - mostly such consumer items as throw-away once they develop a fault - they are designed for cheapness, not a long life, so mostly are not capable of being repaired - at least not for much less than buying a new one.
