"Was" wrong. Past tense of "be". You can say "IF I were to make a wrong decision..." referring to something that has not happened but might, or is considered as possible. But when you are speaking of something that DID happen, you use the past tense (I was, you were, he was, we were, y'all were, they were...). The fact that the same word is used is unfortunate because "were" (plural past) is not the same word as "were" (singular or plural, any person, conditional, hypothetical, subjunctive)"If only I, he, she, it, we, you they, were here...".
