Gone tits up. - this is usually (in the UK anyway) meant as 'it's all gone pear shaped' ~ a plan or orchestrated event has failed, causing everything around it to collapse or disintegrate.

Why tits should go up I don't know. Mine are perfectly happy hanging upsidedown off the nut bag.
