I think it's usually a result of consolidating two or more threads on the exact same subject.

When the problem is objectionable content, I believe the thread just disappears, and does not remain accessible even though "closed."

My opinion, unofficial. I'm sure Joe or a compatriot will chime in with a more definitive answer before long. And then, just maybe, cloe the thread...
