My last colonoscopy was done by two women doctors(well...one, I guess was just a techie) who didn't look like they would welcome jokes...they showed me the whole procedure on a color monitor and confined comments to "hold still"...so I did. Wasn't too bad, and no problems.

If I can't think of anything really clever in such circumstances, I tend to just follow orders and be quiet.
