I'm of the opinion both parties are in urgent need of assessment for mental competence.

Reminds me of a situation I knew of where a couple of borderline mentally disabled people became progressively dirtier and smellier over the course of several weeks, after having been pretty clean for along time previously. Turned out:

Their washer broke down.
She tried to wash clothes in the bathtub, but got frustrated and left them there.
They ran out of clean clothes, and couldn't bathe because the tub was full of smelly wet clothes and water.
Neither of them could figure out what to do, and they were afraid to ask for advice for various reasons.
