I've worked as a professional horticulturist in interior plantscaping. Cacti and succulents were one of our specialties. It's pretty difficult under most non-greenhouse or desert garden conditions to regenerate a cactus after it has declined. It would be best to remove it and replace it. The symptom you describe sounds like over watering, which can happen even in sandy soil.
