So, Kent, if the insurance company is paying a bill for something, or deciding whether to cover me, do they, or do they not have the right to access my medical records in general?
These questions are from a form I was asked to fill out this week:
"Have you or any Proposed Insured been diagnosed or treated for any of the following within the past 5 years:"
(what follows is a loooong list.
Then:
"Other than the above, have you or any Proposed Insured, within the past 5 years: a. Had an electrocardiogram; x-ray; or other special test?(not including HIV tests) b. Been consulted; treated; or examined by any physician or practitioner for any reason not previously mentioned? c. Been operated on, or advised to have any operation? d. Had a physical check-up? e. Been postponed; rated up or declined for Life; Hospitalization; Major Medical; or Accident and Sickness Insurance? f. Made claim for or received benefits or pension due to any injury or illness?"
And, oh yeah, "4.Name, address and phone number of primary care physician:_________________________________________"
BTW, I am glad for your input here as a physician, and please disregard anything in my antagonistic tone. It's certainly not aimed at you : )
