Regarding the stirrups, Jack...childbirth is also far more easily accomplished by a woman squatting (or half standing) rather than lying helplessly on her back with her feet up in stirrups! Ask any midwife. If the woman is upright, then the force of gravity greatly helps her to deliver the baby. If she's lying on her back, gravity can't help her at all. The present barbaric and downright stupid method of delivering babies in modern hospitals is done strictly for the doctors' convenience, not for the mother's....although the majority of doctors probably aren't aware of that at all. They're just doing exactly what they've been taught to do by their profession (which teaches a great many rather odd things). It's convenient for them, because they can easily see what's happening and get access to it. It's not the least bit convenient for their unfortunate patient.
