Have you tried the approach of looking at the cigarette and saying: "Who's stronger? You or me?" If you're stronger, then you don't smoke the cigarette. I knew a guy who managed to quit them in that fashion, and he did it by being stronger than the cigarette each succeeding time that he asked the question...which took awhile...but eventually he didn't have to ask it any longer, and he was done with smoking at that point.
