to my ears that first chord should be d,or d modal
the b minor is too lush ,likewise i do not like f#minor, i am not saying its wrong but not what i would use, neither would i use some of those e minors,
however, Stewart, you have done a good job it is very accurate, good notation of the melody, and your layout is really clear and the chords are good, sometimes chords are a matter of taste and there can be a couple of options.
