If I were to diagnose Trump I would say he does feel shame. You might call him shameless but he knows/feels that he is defective and is always trying to hide and cover it up. He is in fact guiltless as well since guilt is an urge to heal or make amends. He believes in a self importance that makes him unhappy and actually dislikes himself as many Americans do.

He is torn in different directions since he feels the most joy while talking to crowds about his shame that he lies and hides behind his illusion of self importance. Disliking himself compels him to attempt to humilliate others. Many people who hate themselves can identify with Trump.

I tend to call minimizing the ego is the state that is most satisfying when you are doing something that has your complete attention without comparing yourself with others. For some it is music, art or a creativity project.
