It seems from the information and sources given, the early
revival recording of this is the 1963 Ray and Archie Fisher, Edinburgh Festival, subsequently various British performers including the
Steeleye Span in Hark The Village Wait (1970).

I think I heard a performance at a festival in the USA in the early
or mid 1960's.

Did any one stateside perform My Johnny Was a Shoemaker in that period, or know of someone who did or know of a British performer who visited
the US early-mid 60's and performed it ???.
