The News (Sutton Coldfield, Eng.) (Jan. 23, 1909):

"Mr. Harry Millward favoured the company with the song 'Old Macdonald had a farm,' most of those present joining in the chorus."

This is the earliest mention of the title I have found. Canadian and U.S. references begin a few years later.
