(Sometimes it's the easiest of the most obvious chord(s) that don't sound right, or, we can't find them. I've been there myself many times.) I tried to keep the chords pretty basic, although this tune really lends itself to jazzed up passing/transition chords as well. You can also subsitute a Bb9 for the Em7 (on "that once was my heart") and create sort of a walk-down from the C to A7. Anyways, glad it worked out for you.
