Since everything else has failed to restore my voice, I am now scheduled to have another operation just after Indian Neck on May 14th.
This one will consist of an implant of cadaver skin into the gap that was left when they removed the cancer.

Wish me luck, this is probably the last hope.

Karen's Dad did this if you're talking about Cymetra. Its an injectable, cadver skin based, goo that fills the space and a simple procedure they have had good success with for temporary repairs. Or are you doing something newer with the cadaver skin?   All my best thoughts will be with you for a success.

I don't know what the actual procedure looks like, but the Doctor says he has had a lot of success with it. Seems kinda weird to stick a piece of a dead person into my vocal cord, but I'll try anything.

Thanks to everyone - I know that your good wishes mean a lot to Kendall and they certainly do to me as well.

From what the surgeon told us when we were talking about procedures they will implant freeze dried cadaver skin in the hope that

a) Kendall's body won't absorb it, as has been the case with the two previous procedures

b) His own cells will start to grow round the implant and fill the gap naturally.

His surgeon is loathe to use any artificial fillers, in view of the radiotherapy that Kendall had to undergo when the cancer first appeared. That compromised Kendall's immune system and there is always the danger that non-natural fillers could cause an infection that might ultimately lead to the vocal cord having to be removed.

We saw a speech specialist in Boston last year who, as some of you are aware, helped Kendall with voice exercises that led to his being able to perform at the Getaway last November. That guy said that this type of cancer is becoming more prevalent and that there is a lot of research being done to try and restore damaged cords. We're hoping that this procedure works but I ain't giving up hope if it's not a success. I love Kendall just as he is but I know how much it means to him to have a voice restored.

Speaking of Kendall's wrinkled old ass, maybe some cadaver skin would help out there as well. Better yet maybe they just do a complete ass transplant......fix him up with some gigantic ass from someone who passed on from obesity.

Maybe they could also transplant some working genitals as well or at least something that would work without both massive quantities of Mycoxaphalen (generic Viagra) and implanted hydraulics. The last time Kendall blew a line, the hydraulic fluid ruined not just the sheets but the mattress as well and he and Jacqui had to scrub down with GoJo and Lava soap.

Kendall, I heard an interview of Julie Andrews on Fresh Air with Terry Gross, this morning on NPR. You should listen to it. Julie had surgery a few years ago, not for a cancerous growth, but for a cyst on her throat. She'd had her BIG voice since she was little, started performing with it at 7 with her mom and dad in vaudeville, so she really identified with her voice. Anyway, she cannot sing, she can still talk, act, etc. but cannot sing since the surgery. There must be something legal going on about it, because she said the lawyers were not letting her talk about the surgery itself, BUT she had some very positive things to say about all of it...having that voice, losing it and coping, making a life beyond just her singing voice. I am NOT saying it will happen to you, but she was very inspiring to listen to and it made me think of you. One of the things she said it has freed her up to do is write. She has written an autobiography which sounds really good.

Both the previous procedures have used Kendall's own tissue and fat. Each time his body has reabsorbed the filler. I think the idea is that this time there will be nothing that his body can absorb and that, hopefully, his own cells will grow over the implant, giving living tissue to fill the gap.

I don't think that stem cell technology is, as yet, at a stage where they can use it for these sorts of procedures.

There are odd accounts of subsequent changes follwing transplants. My own son in law's experience is one of them- after a kidney transplant from an accident victim from Tennessee ('cadaver' is such a heartless word (no pun intended), imo), for the first time in his life my SIL likes country music. :)

Well, here we are at home on the day that we were supposed to be in Albany, letting the surgeon loose on Kendall's vocal cord again.

Kendall had started having some problems with a cough a few days ago, while we were ranging around, visiting friends before going to a gathering at the weekend in Connecticut. Unfortunately, Connecticut was, according to a local pharmacist, in the midst of the worst pollen season for quite a while and this may have aggravated Kendall's problems.

Suffice to say, when we went to Albany yesterday for him to have pre-admission testing, his surgeon, after looking at chest X-rays, postponed the surgery, and it has been re-arranged for June 11.

This was probably a good thing - after being away from home for two weeks we both wanted nothing more than to be home again and it was a real relief to sink into our own beds at the end of a long day.

Many thanks to those who provided hospitality over the past two weeks. I wish that we had had time to get to visit with more of our friends but time and distance precluded that. There will be another time, however, if I have anything to do with it!

They use only the best cadaver skin. As I understand it, certain types of tissue work better for this surgery than others. According to the National Health website the most desirable comes from the scrotums of deceased winos. The recipient may have some residual side effects such as bad breath and uncontrollable belching but these pass after a few months leaving only a faint aroma of White Rocket.

EBarnacle - I think that the cadaver skin is used because it is an inert, natural substance with less chance of infection or rejection. The body should not reabsorb it either.

We're here in Albany and I've just sneaked out the room leaving Mr Grumpy asleep as long as possible this morning.

We have to be at the clinic at 9.00am and he is to be operated on at 11.00am. Since he is allowed nothing to eat or drink this morning he's going to be a joy to deal with in the meantime. The longer he sleeps now the less time he's going to have to reflect on his fasting!

Kendall is a bit worried about this procedure. Right now it seems to be the last hope for giving him any sort of voice and, after the previous two failures, he is naturally concerned that this one may go the same route.

Lots of good thoughts today please, and for the next few weeks.

I was fine until they took him through to the opering room, then it all got a bit much for a little while.

Kendall's fine - he is on voice rest for five days now. He's a bit tired and grumpy as all get out but, hopefully, that should be resolved by a good meal tonight and a night's sleep. He'll feel a lot better when we're back home in Maine.

Dr Shapshay says that he has put 1cc of this substance in, which he thinks should fill the gap. However, with the scarring from the radiation, it is difficult to get a clear view. He says that there is a little raised 'flap' at the end of the chord, which may also be stopping the chords meeting properly. The plan right now is to see what results we get from this procedure and then whether it's worth going in and shaving off the flap to try and get further closure.

We have to come back in for the post op in about three weeks time so I'm going to try and get that arranged early in the week after Old Songs, when we will already be here.

The surgeon has not closed the door on any further procedures, but it would be good if this one worked.

I'm looking forward to another good meal in the hotel - the food is amazingly good - and a large glass of Chardonnay (for medical purposes, of course!)

He really was loopy - he let me drive all the way home from Albany - not that I was going to let him loose with the car in his present condition.

We stopped in Mass. for a 'comfort break' and, as we got out the car, he told me that he would drive the rest of the way. By the time we got back to the car he had decided that I had better finish the driving. Whatever the painkillers are they're pretty strong!

Wonder of wonders, I've barely heard a peep out of him for 24 hours now! He really is taking the voice rest seriously.

Always happy to join in myself. Shall we just swear for the hell of it or should we pick a particular topic? Maybe prhases said when a mechanical task goes awry like plumbing. Karen will not come around me when I have to fix the plumbing.

Or maybe we need phrases used on a specific thing that gives us a problem. The phrase that Kendall used there might be a good example when speaking to your car.

I heard that years ago! and not since. I was salmon fishing on the Main SW Mirimichi. Heard someone coming along the path next to the river. I heard a thump and knew the person had slipped and hit the muddy ground.

A string of profanity began that was almost poetic, increased upon the second upset and lasted a good 45 seconds until, still waxing poetic, out of the bushes steps Ted Williams (baseball player of note). He was covered in mud from wader boot to shoulder on one side. He arched up upon seeing me and said. "Oh... you're here."

I said, as I was reeling in my line and coming ashore, "That was fucking beautiful. May I watch you fish? Maybe I can learn something new there too." He laughed and said, "Sure sonny." I did learn something. I will never be able to cast a fly like Ted Williams.

About twenty minutes later, another master fly fisher came along. Tom Sellick. I was treated to quite a show of fly fishing technique and finesse. I understand he bought Ted's land. Funny, though... he never calls to invite me fishing.

BTW, that pic of me fishing in the Member Photos & Info is where that happened... on the opposite side, to the right.

Anyway.... how are you feeling today, Cap'n?

I learned to cuss and swear in good company: Boy Scouts of America, Illinois National Guard, US Army, Billy-Bob Shakespeare, Jeff Chaucer, and my two brothers to name but a few of my teachers. I can cuss some in Hebrew, Russian, Arabic, Japanese, Korean, Chinese, Tagalog, Spanish, French, and German thanks to my brothers (and Latin, thanks to the Christian Brothers). I am not sure "maricón" would have been taught in High School, but if you are conversational, I would suggest not using it in polite company...***chuckle***.

Basically this is a play on words foisted on a yanqui who doesn't speak and who is the classic ugly American. The correct thing to say is ".....soy Norteamericano" or "...soy de Norte America" ("I am American" or "I am from the USA". Yes, I am aware that Mexicanos and Canadians are both from North America, but that is the referent in Spanish as spoken in the Americas). But this person, in their ignorant bliss, would think that "maricón" was "American" in Spanish because it has some similarity. Actually it means faggot or homosexual. It is a derisive term in the extreme. Call the wrong person a "maricón" and you likely should be wearing kevlar. I cannot rid my mind of the vision of some gringo standing there, in righteous indignation, proclaiming, "....Yo soy maricón." It is still cracking me up, taken in the context of the story.

There is a voice there - he now has to learn to use it. During the day it has fluctuated from whisper to almost loud at times. His surgeon has told him to start with the exercises that he was doing prior to the strep throat in December and we've got an appointment in Albany for post-op on 1 July, after Old Songs.

Maeve... I bought a 5l (litre... yes, it is spelled "re", Rapaire) jug of Palmolive today. There was a two dollar off coupon at Costco. Besides, I have engineer hands and I must use a gentle dish cleanser to keep away those dishpan hands.

Well, not really... I am just a bargain hungry consumer. And, 5l of dish soap should last me a while. I mostly use it for other household chores, like washing the sides of the dishwasher.

We just got back from Albany where Kendall had his post-op check up.

His surgeon was very pleased with the voice that he heard and the examination showed that the filler has remained in place and is doing the job of almost filling the gap left by the original surgery. Kendall's false vocal cords are coming into play again and he has to do the voice exercises to try and train them not to react but, overall, Dr Shapshay was very pleased with the results.

He says that it will be a few months until we see (and hear) the final results of this procedure and we will be going to see him in October, on the way back from the Getaway.

Kendall will never again have that wonderful voice that I have only heard on recordings - the cord is scarred and stiff and will never vibrate as it should again - but he is getting more of a voice back and should have no problem with storytelling and, with practice, singing songs that would suit the different voice that he will have.
At least he has a voice and that is a real blessing.
