"O Jesu(Jesus) so sweet," "O Jesulein süss" in German, is the one. Sheet music is available under this name, several arrangers.

The hymn is the one by Thilo, posted by Dixon, but arranged for chorale by Bach.
There are a number of translations of the German lyrics, hunt around on the net and you may find the particular one you are looking for. Here are a couple:

Thank you GUEST!! Have been trying to get these three versus right for 35 years. Could always sing two out of the three versus. Had the privelege of singing this old favorite acapela in an old church with my church group while visiting Israel. Love to worship my Lord by singing this song!!
