Anyone know the origin of "The Accident Report"?

I have heard a few people read it, including Johnny Collins. I have a copy of it.

It is an hilarious piece written on the lines of the the "Sick Note" as featured by Gerard Hoffnung on his "Live At The Oxford Union" Recording.

"The Accident Report" is not related in any way to The Sick Note/Bricklayer Letter.

I believe the title of the piece is "Don't Panic! Write a Report."

It is a letter written by the master of a vessel after a series of accidents when it entering harbour, which occur through a simple misunderstanding.

He is writing to the vessels owners explaining the sequence of events.
