Hi, Vijay - It's in the Glory and Praise hymnal from NALR (North American Liturgy Resources), which is now part of OCP (Oregon Catholic Press). I didn't think I'd ever heard the song, but I did find a MIDI here (click), and I found that I have a CD with an instrumental version.

Gosh, haven't sung that for years! Used to do it as a duet with my friend Alison who now lives 150 miles away in a Cathedral!

It's available in the UK too, I think it's a book called 'Acclaim the King', usually found in Christian book shops, worship/music section.

Oddly enough, I heard a pagan version of it at New Year.... but didn't get to see sleeve notes.
