Sibelius drank himself into silence in the 1920s and never finished anything afterwards. I'm pretty sure Michael Tippett didn't smoke and the peak of his career was at the peak of tobacco's popularity in the UK. Shostakovich smoked. The end of his life was a threeway race between lung cancer, heart failure and motor neurone disease.
