The actual origin of this expression (which I had never heard before) come like so many from the world of golf. One day Greg Norman was playing a round in the Canadian Rockies and passed out at the highest point on the course from oxygen deprivation. Understandably as a lowlander Australian/Floridian, he had no high altitude acclimitization. He lay there as following foursomes played through, leaping over his recumbent form. It was all downhill from there. The rest is linguistic history. Or perhaps not.
