The "right the 1st time" part is a problem for me.

Where would the Tele be without the work of Les Paul?

Design, Function Purpose part of the thread is fun though.

How about The Zippo Lighter, The Skidoo, and the Saturn SL1?

I have the Saturn. It is an amazingly well built car which GM let slide into oblivion due to corporatism.

Female body? I donno. Tell that to a woman who has had cancer, or for that matter a C-section. Well designed, maybe, but far from flawless in design and purpose.

Only "first time" if you assume the designer didn't know about previous attempts.
