The last name: Take the first 2 letters of your actual last name, and add the first 2 letters of the city (or place) where you were born. I should be Chaja Hato, but I cheated, because I wanted to include my middle names, and I used my legal last name because (for once!) it sounded better. So my Star Wars Name is Chaja Rade Br'Voto.
